OSHA is reportedly conducting an investigation following a fatal accident involving a 59-year-old worker at a manufacturing plant in South Macon. The worker apparently died in October when a robotic arm knocked him onto a conveyer belt, causing him to be crushed by pallets.

The plant in question has a history of injuries, including six amputations, and a worker death in 2008. In 2014, the plant was placed in the severe-violation enforcement program intended for employers who repeatedly and willfully fail to follow OSHA standards.

The Nichiha USA plant, owned by a Japanese corporation, apparently manufactures fiber-cement panels and has been cited by OSHA over 60 times since it opened in 2007. According to OSHA, many of the violations were classified as serious and some were repeat violations. One violation referred to the plant's failure to use a group lockout procedure to prevent equipment from suddenly starting up without warning. The worker who was recently killed had gone through OSHA training at another facility, and his nephew believes that the accident occurred because of the plant's failure to properly train its workers on this procedure.
